Job summary

Creative Solutions In HealthCare in Texas is seeking a full-time Occupational Therapist (OT) to help residents improve independence and safety through meaningful activities and ADLs.

You will evaluate patients, develop individualized treatment plans, coordinate with the interdisciplinary team, document thoroughly, and support families and caregivers while meeting state and federal regulations.

Qualifications

• Active Texas OT license required.
• SNF or long-term care experience preferred.
• Knowledge of OT treatment practices and documentation.

Responsibilities

• Consult with patients to evaluate their condition and impact on daily living.
• Develop individualized treatment plans to maximize ADL/IADL performance.
• Assess home environments and recommend modifications.
• Provide training on adaptive equipment and safety.
• Document thoroughly and timely per policy and regulations.
• Collaborate with the interdisciplinary team to ensure quality care.
