Job summary

Creative Solutions In HealthCare is seeking a dedicated Occupational Therapist (OT) to join our in-house team. You will help residents improve independence, safety, and quality of life through meaningful daily activities and functional outcomes.

You will collaborate with an interdisciplinary team, document care thoroughly, and support patients and families with education and resources in a SNF/long-term care setting.

Qualifications

• Active and valid Texas Occupational Therapy license.
• In good standing with regulatory agencies and licensing boards.

Responsibilities

• Consult with patients to evaluate their condition and impact on daily activities.
• Develop individualized treatment plans for ADLs/IADLs.
• Assess home environments and recommend modifications.
• Recommend adaptive equipment and provide training.
• Deliver treatments per plan of care and monitor progress.
• Collaborate with interdisciplinary team for high-quality care.
• Document thoroughly and timely per policies and regulations.
• Attend required meetings with Director of Rehab.
