Job summary

Creative Solutions in Healthcare - East is seeking a dedicated Full-Time Occupational Therapist (OT) to join our in-house rehab team. The OT will support residents’ independence and quality of life through meaningful daily activities and functional outcomes.

Responsibilities include evaluating patients, developing individualized treatment plans, modifying environments, training on adaptive equipment, and collaborating with an interdisciplinary team.

Qualifications

• Active and valid Texas Occupational Therapy license.
• Experience in SNF or long-term care preferred.
• Knowledge of occupational therapy treatment practices.

Responsibilities

• Consult with patients to evaluate physical condition and impact on daily occupations.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans to maximize ADLs/IADLs.
• Assess home environments and recommend modifications.
• Recommend adaptive equipment and provide training for independence and safety.
• Deliver treatments per plan of care and collaborate with the team.
• Monitor response to treatment and adjust interventions as needed.
• Provide clinical education to patients, families and caregivers.
• Collaborate with interdisciplinary team for high-quality care.
• Complete documentation thoroughly and timely per policies.
• Comply with all regulations and attend required meetings.
