Qualifications

• Active Texas OT license in good standing
• Experience in SNF or long-term care settings preferred
• Knowledge of OT evaluation and treatment techniques

Responsibilities

• Evaluate patient condition and impact on daily occupations
• Develop individualized ADL/IADL treatment plans
• Assess living environments and recommend modifications
• Provide training on adaptive equipment and safety
• Collaborate with interdisciplinary team to adjust plan of care
• Document thoroughly and timely per policies and regulations
• Attend required meetings with the Director of Rehab
