Job summary

Creative Solutions In HealthCare in Texas seeks a compassionate Occupational Therapist (OT) to join our in-house rehab team on a PRN basis. You will evaluate residents, develop individualized ADL/IADL plans, and help maximize independence and safety.

This role emphasizes collaboration with an interdisciplinary team, documentation accuracy, and adapting care to varying patient needs. A current Texas OT license and SNF or long-term care experience are preferred; Net Health knowledge is a plus.

Qualifications

• Active and valid Texas Occupational Therapy license
• In good standing with regulatory agencies
• SNF or long-term care experience preferred
• Current knowledge of occupational therapy treatment practices
• Experience with Net Health Electronic Documentation System preferred
• Ability to manage patients with varying personalities and needs
• Strong communication, collaboration, and clinical decision-making skills

Responsibilities

• Consult with patients to evaluate their physical condition and its impact on daily occupations, well-being, and quality of life
• Assess and interpret patient evaluations and test results
• Develop individualized treatment plans using a variety of therapeutic techniques to maximize performance in ADLs and IADLs
• Assess patients’ home and living environments and recommend modifications based on their needs and capabilities
• Recommend adaptive equipment and provide training to support independence and safety
• Ensure all treatments are delivered in accordance with the established plan of care
• Monitor patient response to treatment and modify interventions as clinically appropriate in collaboration with the interdisciplinary team
• Provide clinical education and support to patients, family members, and caregivers
• Collaborate with interdisciplinary team members to ensure high-quality patient care
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations
• Comply with all applicable rules, regulations, and procedures
• Attend required meetings as designated by the Director of Rehab
