Job summary

Creative Solutions In HealthCare in Arlington, TX is seeking a dedicated full-time Occupational Therapist (OT) to help residents improve independence, safety, and quality of life through meaningful daily activities and functional outcomes.

You will provide person-centered care and collaborate with an interdisciplinary team in a skilled nursing/long-term care setting. A current Texas OT license and experience with Net Health documentation are preferred.

Qualifications

• Active Texas Occupational Therapy license in good standing.
• Experience in Skilled Nursing Facility (SNF) or long-term care preferred.
• Ability to collaborate with an interdisciplinary team and document care accurately.

Responsibilities

• Consult with patients to evaluate their condition and impact on daily activities.
• Develop individualized treatment plans to maximize ADL/IADL independence.
• Assess home environments and recommend safe modifications.
• Provide training on adaptive equipment and safety practices.
• Document thoroughly in the Net Health system in compliance with policies.
