Job summary

Creative Solutions In HealthCare is seeking a dedicated Occupational Therapist to join our in-house team in a skilled nursing facility setting in Texas. You will focus on improving residents' independence and safety through meaningful daily activities and functional goals.

Responsibilities include evaluating patients, developing treatment plans, modifying interventions with the interdisciplinary team, and documenting thoroughly in accordance with policies. SNF experience preferred.

Qualifications

• Active Texas OT license.
• SNF or long-term care experience preferred.
• Knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities.

Responsibilities

• Consult with patients to evaluate their condition and its impact on daily occupations.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans to maximize ADLs/IADLs.
• Assess home environments and recommend modifications for safety.
• Recommend adaptive equipment and provide training.
• Monitor response to treatment and adjust interventions with the team.
• Complete documentation thoroughly and in line with plan of care and regulations.
