Prosper Health is seeking an experienced MDS Coordinator to manage patient assessments, schedules, and care plans in a patient-centered Skilled Nursing environment in Florida. The role focuses on accurate MDS submission, regulatory compliance, and coordinating with the IDT to optimize resident outcomes.

The ideal candidate holds a Nursing degree with an active Florida RN license, has strong MDS 3.0 proficiency, and at least two years of LTC experience, with prior MDS coordination experience.

Formación

• Bachelor’s in Nursing and an active Florida RN license.
• Proficiency with MDS 3.0 and related regulatory requirements.
• Minimum two years of clinical LTC experience.
• Knowledge of state and federal regulations governing MDS.
• Prior experience as an MDS coordinator.

Responsabilidades

• Oversee completion of MDS, CAAs and Care Plans per regulations.
• Coordinate care plans and ARD with the IDT and families.
• Ensure resources are available and patient care meets standards.
• Ensure timely electronic submission of all MDS to the state database.
• Identify regulatory risks and communicate with Administrator and DON.
• Educate IDT on RAI processes and train staff as needed.
• Analyze QI/QM data to identify monthly trends.
• Participate in committees and support quality initiatives.
