Prosper Health & Rehab in Florida is seeking a qualified MDS Coordinator to oversee patient assessments, ensure timely schedules, and maintain accurate MDS, CAAs, and care plans in compliance with federal and state guidelines.

You will collaborate with the IDT, administrators, and nursing leadership to optimize resident outcomes, coordinate Medicare/Medicaid processes, and support high-quality, compliant care across the facility.

Qualifications

• Bachelor’s in Nursing.
• Proficiency in MDS 3.0.
• Demonstrating knowledge of state and federal regulations.
• Registered Nurse with active license in the state employed.
• Minimum two years of clinical LTC experience.
• Prior experience as an MDS coordinator.

Responsabilités

• Oversees accurate and thorough completion of the MDS, CAAs and Care Plans.
• Communicates with Administrator and Director of Nursing about regulatory risk and clinical trends.
• Captures the ARD and determines RUG score for care and services.
• Ensures timely electronic submission of all MDS to state database.
• Leads care management processes with IDT and family to resolve discharge barriers.
