Job summary

CareRite Centers, LLC is seeking an MDS Coordinator (RN) to join our skilled nursing facility team in Florida. The role focuses on accurate MDS, CAA, and care plan completion in compliance with federal and state guidelines, while supporting patient transitions and discharge planning.

The ideal candidate holds an active FL RN license with at least two years of clinical experience and one year in long-term care; prior MDS coordination experience is preferred. Great benefits offered.

Qualifications

• Registered Nurse with active FL license.
• Minimum two years of clinical experience in a health care setting.
• Minimum one year of experience in a long term care setting.
• Prior experience as an MDS coordinator preferred.

Responsibilities

• Oversee accurate completion of MDS, CAAs and Care Plans per regulations.
• Understand MDS requirements for Medicare, Managed Care and Medicaid.
• Facilitate care management to identify discharge barriers and ensure safe transitions.
• Analyze QI/QM data with the Director of Nursing to identify trends.
• Ensure timely Utilization Review and Triple Check.
• Participate in facility committees as required.
