Job summary

• Active RN license required.

• Strong understanding of MDS 3.0, RAI, and regulatory requirements.
• Skilled in EHR systems and documentation.

Responsibilities

• Lead and manage timely, accurate MDS assessments and care plans.
• Ensure full compliance with state/federal guidelines.
• Partner closely with clinical teams to support individualized, resident-centered care.
• Review and audit documentation for accuracy and completeness.
• Provide guidance and training on MDS processes when needed.
