Signature HealthCARE of Chapel Hill is seeking a caring LPN/LVN to provide direct nursing care to residents in a 108-bed setting in Cary, NC. You will administer medications, perform wound care, support ADLs, and supervise nursing assistants while upholding resident dignity and safety.

Qualifications

• Active LPN/LVN license in state of practice.
• CPR/BLS certification preferred.
• At least 1 year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with Electronic Health Records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) while maintaining dignity and safety.
• Supervise and direct nursing assistants and medication aides during shifts per policy.
• Monitor residents’ conditions and notify physician or family of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per facility policy.
• Coordinate admissions, transfers, and discharges.
• Participate in QA activities and staff education.
