Signature HealthCARE of Chapel Hill is seeking an experienced LPN/LVN to provide direct nursing care to residents in a 108-bed facility. You will administer medications, perform wound care, and support overall patient comfort and safety.

You will supervise nursing assistants, contribute to care planning with the interdisciplinary team, maintain accurate documentation, and coordinate admissions, transfers, and discharges in a team-driven environment.

Competenze

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with EHR systems and Microsoft Office applications.

Mansioni

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
