Signature HealthCARE of Chapel Hill is seeking a dedicated LPN/LVN to provide direct nursing care to residents in a 108-bed facility. You will supervise nursing assistants, manage ADLs, administer medications, and document care within a patient-centered team environment.

Experience in long-term care and familiarity with EHR systems are preferred. The role supports a compassionate culture with opportunities for professional growth and development within Signature HealthCARE.

Competenze

Current, active LPN/LVN license in the state of practice.
CPR/BLS certification preferred (American Heart Association or American Red Cross).
Minimum one (1) year of clinical nursing experience; long-term care experience preferred.
Experience in a supervisory or charge nurse role preferred.
Familiarity with electronic health records (EHR) systems and MS Office.

Mansioni

Provide direct nursing care to residents, including medication administration and wound care.
Assist residents with activities of daily living (ADLs) with dignity and safety.
Supervise nursing assistants and medication aides during assigned shifts.
Monitor residents’ condition and notify physician of changes.
Develop and review individualized care plans with interdisciplinary team.
Maintain accurate clinical documentation and coordinate admissions/transfers.
