Signature HealthCARE of Chapel Hill is seeking a caring LPN/LVN to provide direct nursing care in a 108-bed facility. You will administer medications, perform wound care, and assist residents with daily activities to promote dignity and comfort.

The role requires an active North Carolina LPN/LVN license, CPR/BLS preferred, and at least 1 year of clinical nursing experience (long-term care preferred). Strong documentation and communication skills are essential.

Formación

• Active LPN/LVN license in NC.
• CPR/BLS certification preferred (AHA/Red Cross).
• Minimum 1 year of clinical nursing experience; long-term care preferred.
• Experience in supervisory/charge nurse role preferred.
• Strong documentation and medication administration skills.
• Knowledge of Federal and State long-term care regulations.

Responsabilidades

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with ADLs while ensuring comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during shifts.
• Monitor residents' status and notify physicians of changes.
• Develop, implement, and review individualized care plans with the team.
• Complete timely clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
