Signature HealthCARE of Chapel Hill is seeking an LPN/LVN to provide direct nursing care, supervise nursing staff, and manage resident comfort and safety. The role emphasizes medication administration, ADL support, and careful documentation within an interdisciplinary team.

Ideal candidates hold an active LPN/LVN license, CPS/BLS is preferred, and have 1+ year of clinical nursing experience, preferably in long-term care. EHR familiarity and MS Office skills are valued.

Qualifikationen

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Aufgaben

• Provide direct nursing care to residents, including medication administration and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during the shift.
• Monitor and observe residents' status and communicate changes to the physician or responsible party.
• Develop and review individualized resident care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges per procedures.
• Participate in quality assurance and staff education.
