Job summary

Signature HealthCARE of Chapel Hill is seeking a caring LPN/LVN to provide direct nursing care in a 108-bed facility in North Carolina. You will manage medications, wound care, and daily living activities while supervising nursing assistants and ensuring patient safety.

Qualifications

• Active LPN/LVN license in North Carolina.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs to ensure comfort, dignity and safety.
• Supervise nursing assistants and medication aides during the shift.
• Monitor residents' status and notify physicians of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges per procedures.
• Participate in QA activities, staff education and regulatory surveys.
