Job summary
Longevity Health is seeking a Nurse Practitioner or Physician Assistant to manage a panel of enrolled members in long-term care settings, delivering collaborative primary care and leading ISNP care activities. You will develop individualized care plans, participate in care team meetings, and communicate regularly with families and PCPs to align goals.
Candidates should have an MSN or related Master’s degree, NP/PA licensure, DEA license, and experience in nursing homes; a valid driver’s license
Qualifications
- 6+ months experience as Nurse Practitioner or Physician Assistant.
- MSN as NP or Master’s degree from an accredited program.
- Experience in nursing home environments strongly desired.
- Must have active NPI and Medicare provider numbers.
- DEA license required.
- Valid state driver’s license and auto insurance required.
Responsibilities
- Provide collaborative primary care to a panel of members.
- Lead ISNP care activities and advanced care planning.
- Coordinate with health plan operations, reviews, and quality gaps.
- Communicate with PCPs, families, and care teams.
- Manage member caseloads and support market goals.
- Represent Longevity Health in nursing facilities.
