Longevity Health is seeking a Nurse Practitioner or Physician Assistant to manage a panel of enrolled members in long-term care facilities. You will conduct comprehensive assessments and develop individualized care plans, while participating in care team meetings and maintaining open communication with families and staff.

The role requires state NP/PA licensure, DEA license, active NPI and Medicare provider numbers, and prior nursing home experience.

Qualifikationen

• 6+ months experience as a Nurse Practitioner or Physician Assistant
• Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program
• Experience working in the nursing home environment is strongly desired
• Computer/Technology proficiency required
• The ability to engage and communicate effectively with physicians, members, and family members
• Self-directed but functions well in a team environment
• Excellent clinical skills—assessment, diagnosis, treatment, prescribing specifically for the chronically ill population
• Must have an active national provider number (NPI) and an active Medicare provider number
• DEA License required.
