Job summary

PATH is seeking a dedicated Case Manager for the Marmion Way site in Los Angeles, CA. This role supports clients from intake through housing planning, coordinating health and social services, and navigating the CES to secure stable housing.

The position emphasizes individualized service planning, crisis intervention, and connection to community resources, with a focus on equity and social justice within California programs.

Responsibilities

• Conducts intake and develops ISP addressing housing goals and income.
• Provides referrals and connects clients to resources for ISP goals.
• Completes program assessments (e.g. VI-SPDAT).
• Maintains documentation per PATH policies and HIPAA.
• Develops therapeutic relationships and provides crisis intervention.
• Uses motivational interviewing and trauma-informed care.
• Maintains knowledge of community resources to wrap services.
