Job summary

Health Connect America seeks an Integrated Care Coordinator to plan, coordinate, and monitor individualized treatment plans in behavioral health. You will support the Nurse Practitioner with on-site tasks and facilitate enrollment for clients.

You will engage in outreach for referrals, track HEDIS gaps, and ensure timely documentation while collaborating with care teams and community partners.

Qualifications

• TN: Bachelor’s degree required; human services preferred.
• NC: Various licensure options or relevant degrees with experience required.
• Two years of experience with behavioral health populations emphasized.

Responsibilities

• Coordinate and monitor integrated care plans aligned with behavioral health goals.
• Assist Nurse Practitioner with clinic documentation and on-site facilitation.
• Engage in outreach for new referrals and educate about programs.
• Monitor regulatory compliance and maintain medical records promptly.
• Coordinate transitions between care settings and discharge plans.
• Attend treatment team and supervision meetings and liaise with partners.
