Job summary

Health Connect America in North Carolina seeks an Integrated Care Coordinator to connect clients, providers, and community resources, ensuring coordinated behavioral and physical health support and smooth transitions. You will manage care planning, monitor services, document care, and collaborate with treatment teams and Nurse Practitioners to promote client well-being.

This role emphasizes person-centered care, adherence to policies, and engagement with marketing and referrals to grow program

Qualifications

• Bachelor's Degree in Human Services is required, with experience working with youth or families.
• Qualified Professional (QP) with experience serving children/adolescents with a primary mental health diagnosis is required.

Responsibilities

• Oversee care planning, coordination, and monitoring for assigned clients in behavioral and physical health.
• Coordinate and monitor services across treatment teams, primary care providers, and community resources to ensure seamless care delivery.
• Provide health education, guidance, and crisis support to clients and families as needed.
