Job summary

Health Connect America is seeking an Integrated Care Coordinator to deliver comprehensive, person-centered care by planning, coordinating, and monitoring individualized treatment plans. You will assist the Nurse Practitioner with clinic documentation and support marketing efforts to grow referrals, while ensuring high-quality care across our multi-state network.

Qualifications

• Bachelor’s Degree in any discipline required; preference for human services or related fields.
• Experience working with children and families in case management or community resource positions.
• State-specific regulations may apply for TN/NC roles.

Responsibilities

• Deliver comprehensive, person-centered care by planning, coordinating, and monitoring individualized treatment plans.
• Assist Nurse Practitioner with clinic documentation and on-site facilitation.
• Support marketing initiatives for new client referrals and outreach to integrated care members.
• Maintain medical record compliance and timely documentation of care coordination activities.
• Coordinate follow-up services and transitions of care between settings.
• Attend Treatment Team and supervision meetings and act as liaison with professionals and agencies.
