Job summary

Washington County Mental Health Services seeks Integrated Care Navigators to help clients and providers navigate administrative, referral, and follow-up systems, using the EMR to manage records and referrals.

You will support outreach, missed appointment protocols, Medicaid or benefits applications, and coordinate care with primary care and community resources to ensure seamless service delivery.

Qualifications

• Care coordination or case management experience.
• Familiarity with Medicaid and community referrals is highly desirable.
• Strong verbal and written communication skills.

Responsibilities

• Complete, track, and follow up on client referrals in the EMR for health, housing, crisis, employment, and specialty services.
• Review client charts, obtain required ROIs, and maintain accurate care coordination notes in the EMR.
• Identify clients eligible for Medicaid, Medicare, SNAP, or other benefits and assist with the application process.
• Execute outreach and missed appointment protocols, track care pathway adherence, and address access barriers with clients and providers.
• Participate in staff huddles and coordinate care across interdisciplinary teams and external partners.
• Assist with intake activities, including preparing client charts, routing information, recording health history, and collecting vital signs.
