Job summary

Avenue360 Health and Wellness in Houston, TX, seeks an Outreach Navigator/Assessor to identify and connect individuals experiencing homelessness with housing resources through CES. You will conduct outreach, perform HMIS-based housing assessments, and provide pre-navigation support for PSH and RRH programs.

Responsibilities include community outreach, documentation, and collaboration within a multidisciplinary team to meet housing-placement goals, with emphasis on serving vulnerable populations

Qualifications

• Bachelor’s degree in social work, Human Services, Public Health, or related field, or equivalent experience.
• Valid driver’s license preferred.
• Bilingual (English/Spanish) preferred.
• Minimum two years of experience in social services, housing programs, homelessness outreach, or case management.
• Experience working with vulnerable populations including individuals experiencing homelessness.
• Strong client engagement, outreach, and housing navigation skills.
• Competency in data entry and documentation within HMIS or similar systems.
• Strong communication, problem-solving, and interpersonal skills.
• Ability to manage multiple priorities while maintaining accurate documentation.
• Ability to work both independently and in a team.

Responsibilities

• Conduct outreach and complete client enrollments and housing assessments through CES.
• Provide pre-navigation services to obtain eligibility documentation for PSH and RRH.
• Commit to at least four hours per week of community outreach activities.
• Provide transportation and logistical support for housing-related activities (identifications, SSN cards, etc.).
• Attend required Housing Assessor, Housing Navigator, and HMIS trainings to effectively use CES.
