Avenue 360 in Houston seeks an Outreach Navigator/Assessor to identify and connect homeless individuals with housing resources via CES. You will conduct outreach, complete housing assessments, and help clients obtain required documentation for PSH and RRH.

You will dedicate weekly hours to community outreach, and provide transportation and documentation support. Training in HMIS and CES procedures is included to ensure effective navigation.

Qualifications

• Bachelor’s degree in social work, Human Services, Public Health, or related field or equivalent experience.
• Valid driver’s license is preferred (for outreach and transportation duties).
• Experience in social services, housing programs, homelessness outreach, or case management is required.

Responsabilités

• Conduct outreach and enroll clients in housing assessments through the Coordinated Entry System (CES).
• Provide pre-navigation services to obtain eligibility documentation for Permanent Supportive Housing (PSH) and Rapid Rehousing (RRH).
• Dedicate at least four hours per week to community outreach activities, including community calls and encampment reconnaissance.
• Provide transportation and logistical support for housing-related activities (ID docs, SSN cards, disability verification, housing verification, SSI letters).
• Attend Housing Assessor, Housing Navigator, and HMIS trainings to use CES effectively.
