Job summary

Avenue360 Health and Wellness in Houston seeks an Outreach Navigator/Assessor to connect individuals experiencing homelessness with housing resources through the Coordinated Entry System (CES). This role conducts outreach, completes housing assessments through HMIS, and provides pre-navigation services to assist for PSH and RRH programs.

Qualifications

• Bachelor's degree or related field equivalent experience.
• Valid driver's license preferred for outreach and transportation duties.
• Bilingual English/Spanish preferred.

Responsibilities

• Conduct outreach and enrollments through the Coordinated Entry System (CES).
• Complete housing assessments in HMIS and coordinate housing resources.
• Provide pre-navigation services to obtain eligibility documents for PSH and RRH.
• Dedicate a minimum of 4 hours per week to community outreach activities.
• Assist clients with transportation, IDs, SS cards, and other housing documents.
