Job summary

Avenue 360 Health and Wellness is seeking an Outreach Navigator/Assessor (Heights) to identify and connect individuals experiencing homelessness with housing resources through the Coordinated Entry System (CES). The role conducts outreach, performs housing assessments in HMIS, and provides pre-navigation support to obtain essential documentation for PSH and RRH programs.

Qualifications

• Bachelor's degree in social work, Human Services, Public Health, or related field, or equivalent experience.
• Valid driver's license preferred for outreach and transportation duties.
• Minimum two years of experience in social services, housing programs, homelessness outreach, or case management.
• Experience with vulnerable populations and housing navigation.
• Proficiency with HMIS or similar case management systems and documentation.
• Strong communication, problem-solving, and interpersonal skills.
• Ability to manage multiple priorities and work independently or in a team.
• Proficiency with Microsoft Office Suite (Word, Excel, Outlook).

Responsibilities

• Conduct outreach and complete enrollments and housing assessments for individuals through the Coordinated Entry System (CES).
• Provide pre-navigation services to obtain eligibility documentation for PSH and RRH programs.
• Dedicate a minimum of four hours per week to community outreach activities, including calls and encampment reconnaissance.
• Provide transportation and logistical support for housing-related activities (ID docs, SSN, disability verification, homeless verification, SSI letters).
• Attend required Housing Assessor, Housing Navigator, and HMIS trainings to use the Coordinated Entry System.
