Job summary

Mental Health America of Los Angeles (MHALA) is seeking a Personal Service Coordinator to assess and engage potential members for Housing Supportive Service Programs. You will apply intensive case management to help individuals access services and transition to appropriate care levels based on psychosocial rehab models.

Required: high school diploma; 1+ year in psychosocial rehab; preferred bachelor's in a related field.

Qualifications

• High school diploma or GED required.
• Minimum of one year of experience providing psychosocial rehab related services to a disabled population.
• Bachelor's degree in a human related field preferred.
• Must possess a high level of tolerance, patience, and understanding for individuals with multiple needs in a busy work atmosphere.
• Must meet target billing and productivity goals – 85 hours a month for full-time staff.

Responsibilities

• Outreach and engage and enroll new members referred to the program. Develop a Recovery Plan and/or Personal Service Plan with each member.
• Mediate and advocate to help members obtain housing, health care benefits, entitlements, etc.
• Visit members at home, hospital, or jail as needed to support housing stability and reduce homelessness.
• Transport members to appointments and errands when appropriate.
• Provide coaching and support in independent living skills and daily routines.
