Job summary

Housing Forward in the United States is seeking a Housing and Health Case Manager Supervisor to coach case managers, strengthen housing and health partnerships, and monitor progress toward program goals.

You will oversee data entry into HMIS and manage staff, participating in audits and coordinating with partners to improve service delivery. Strong leadership and organizational skills are essential for success in this role.

Qualifications

• MSW or related master's degree with minimum three years supervisory experience.
• Bachelor’s degree with four years’ supervisory experience.
• Demonstrates ability to supervise a team of staff with different responsibilities.
• Demonstrates organizational skills.
• Willingness and ability to interact with external partners/funders and able to represent agency’s perspectives.

Responsibilities

• Supervise case managers and monitor progress toward program objectives.
• Lead biweekly staff staffing sessions.
• Conduct interviews, hire and onboard new personnel.
• Perform annual performance evaluations and coach staff.
• Maintain timely HMIS data entry and client records.
• Coordinate health and housing projects with partners.
