The role offers full-time hours with hourly classification and a salary range of $24.00–$24.50 per hour.

Qualifications

• Direct service to chronically homeless individuals.
• Ability to develop service plans and coordinate referrals.
• Engage clients to support housing stability.

Responsibilities

• Provide intensive case management Housing First services to chronically homeless persons to maintain Permanent Supportive Housing.
• Conduct intake and assessment of barriers, service plan development, linkages to resources.
• Assist with leasing up and moving into housing.
