Job summary

Elite Hospice, part of LHC Group and aligned with the Optum family, seeks a Registered Nurse Case Manager to oversee the Plan of Care for hospice patients in the community.

You will assess needs, develop and implement care plans, and coordinate with the IDG to deliver compassionate, high-quality end-of-life care while maintaining compliance and thorough documentation. This role is located in Hot Springs, Arkansas and requires RN licensure and a valid driver's license.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation for hospice eligibility within 48 hours of referral.
• Identifies patient/family needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care with the IDG.
• Documents problems, goals, interventions, and responses to care.
• Collaborates with patient/family, attending physician and IDG for daily care.
