Job summary

Amedisys is seeking a PRN Nurse Practitioner to provide home-based patient care across a range of conditions. The role includes 125 per visit and 75 admin rate, with benefits such as paid time off, telehealth, and a 401(k) plan.

Qualifications

• Current and unencumbered NP license in the state of employment.
• Minimum of one year as a Nurse Practitioner or hospice RN in last three years.
• Authority to practice as a NP in the state of services.
• Authority to prescribe as a NP in the state of services.
• Must have Medicare billing privileges as NP.
• Must meet state NP collaborative requirements and provide physician agreements.
• Must carry state-required NP malpractice/insurance.

Responsibilities

• Communicate significant findings or changes in condition to the clinical manager and team.
• Complete face-to-face encounters and assess disease progression, function, and quality of life.
• Order and review labs and diagnostics; document treatment plan and review results with physician.
• Make referrals to other disciplines as indicated by patient needs.
• actively participate in interdisciplinary team conferences.
