Job summary

Amedisys is seeking a Nurse Practitioner to provide hospice and home health care in Tennessee. You will work with clinical teams to assess patients, manage symptoms, and coordinate care in patients’ homes.

Responsibilities include conducting face-to-face encounters, ordering labs and imaging as needed, and participating in interdisciplinary team conferences. Requires NP licensure, Medicare billing privileges, and CPR certification; prior hospice/home health experience is preferred.

Qualifications

• Must hold an active Nurse Practitioner license in the state of assignment.
• Minimum of 1 year experience as a Nurse Practitioner or hospice RN within the last 3 years.
• Must be authorized to practice and to prescribe as an NP in the service state.
• Must have Medicare billing privileges as an NP.
• Must meet state-specific collaborative requirements and provide physician agreements; indicate transferability.
• Must carry insurance as required by state law for NP practice.

Responsibilities

• Communicate findings and changes in condition to the clinical team.
• Conduct face-to-face encounters and assess disease changes, function decline, pain and symptom management, and quality of life.
• Order and review lab tests, X-rays, EKGs, and other diagnostics; document treatment plans.
• Make referrals to other disciplines as indicated; participate in interdisciplinary conferences.
