Job summary

OneWell Health Care is seeking a Home & Community Support Professional in Indianapolis, IN to deliver person-centered services under BDS guidelines. The role focuses on independence, daily living skills, health and safety, and community integration for individuals with IDD.

The HCSP will work in home and community-based settings, supporting personal care, behavior support, documentation, and collaboration with families and supervisors to ensure high-quality care and compliance with policies.

Qualifications

• Ability to complete electronic documentation and daily service notes.
• Maintain accurate records and reports.
• Understand incident reporting processes.
• Understand infection control and universal precautions.

Responsibilities

• Provide direct care and person-centered support to individuals with IDD in home and community settings.
• Assist with personal care, meal preparation, mobility, and daily living skills.
• Monitor health, behavior, and safety; report concerns and implement interventions.
• Complete daily service notes and coordinate with families, guardians, and interdisciplinary teams.
