Job summary

OneWell Health Care is seeking a Home & Community Support Professional (HCSP) to provide direct, person-centered support to individuals with IDD in home and community settings across Indiana. You will implement PCISP goals, assist with daily living activities, and monitor health and safety while maintaining thorough documentation.

The role emphasizes caregiver responsibilities in field-based settings, with flexible scheduling and strong teamwork with families and the interdisciplinary team.

Qualifications

• High school diploma or equivalent required.
• CPR Certification required.
• Negative TB test required.
• Criminal Background Check required.

Responsibilities

• Provide direct care and person-centered support to individuals with IDD in home and community settings, following PCISP.
• Assist with personal care, meal preparation, mobility, and daily living skills.
• Monitor health, behavior, and safety; report concerns.
• Maintain accurate daily documentation and service notes.
• Collaborate with families, guardians, supervisors, and interdisciplinary teams to ensure quality care.
