Job summary

Accra is seeking a Basic 245D Service Coordinator to manage participant-directed licensed services in Minnesota. You will meet with clients, set up services, monitor adherence, and explain DSP timecard processes while coordinating with designated representatives and county case managers.

The role requires staying current with Minnesota rules and collaborating with Accra staff to improve service delivery and outcomes for people with disabilities and the elderly.

Qualifications

• Bachelor-equivalent education in human services, education, or health, with direct care experience preferred.
• Alternative credentials acceptable with substantial direct care experience with disabilities or elderly.
• Multiple pathways exist to meet qualifications (degree, diploma, or training plus experience).
• Candidates should demonstrate knowledge of services for people with disabilities and the elderly in Minnesota.

Responsibilities

• Meet with and set up services for participants under Basic 245D licensing.
• Monitor ongoing services and explain Direct Support Staff employment and timecard processes.
• Determine Responsible Party and DSP competency; maintain communication with clients, representatives, and county case managers.
• Stay updated on policy changes and participate in relevant committees or meetings.

Four-year degree in human services, education, or health

Two-year degree in human services, education, or health

Diploma in human services, education, or health

Minimum of 50 hours of education and training in human services
