Job summary

ACCRA Management Group LLC is seeking a Basic 245D Service Coordinator in Minnesota. The role focuses on meeting with participants, setting up and monitoring licensed Basic 245D services, and guiding Direct Support Staff employment and timecard processes.

Candidates will communicate with clients, designated representatives, and county case managers. They must stay current on Minnesota rules for Basic 245D services and participate in related committees for ongoing knowledge growth.

Qualifications

• Bachelor-level or higher education in human services/education/health with applicable direct care experience.
• Alternate paths include a two-year degree with relevant direct care experience.
• Postsecondary diploma plus three years of relevant direct care experience accepted.
• Minimum 50 hours of related education plus four years of direct care experience under supervision.

Responsibilities

• Meet with participants to assess needs and preferences for Basic 245D services.
• Set up and monitor licensed Basic 245D services as directed by Minnesota regulations.
• Explain Direct Support Staff employment and timecard processes to participants and representatives.
• Maintain communication with clients, their representatives and county case managers; provide ongoing service monitoring.

Education

Four-year degree in human services/education/health + 1 year direct care experience

Two-year degree in human services/education/health + 2 years direct care experience

Postsecondary diploma in human services/education/health + 3 years direct care experience

50 hours of education/training in human services + 4 years direct care experience under qualified supervision
