MBK Senior Living in Huntington Beach, CA is seeking a Health Services Coordinator (LVN/LPN) to support the health services team with administrative and clinical duties under the Director of Health Services. The role emphasizes resident dignity, independence, and high-quality care.

Requirements include an active LVN/LPN/RN license, 2+ years in a healthcare setting, and strong communication and documentation skills.

Qualifications

• Must hold an active LVN/LPN/RN license in good standing.
• Minimum 2 years’ experience in a healthcare setting.
• Strong communication, organizational, and problem-solving skills.

Responsabilités

• Conduct and coordinate assessments/evaluations for potential residents and make admission recommendations.
• Draft and update Individualized Service Plans (ISP).
• Review service plans with relevant parties and the Executive Director.
• Provide training on medication delivery, safety protocols, and state requirements.
• Perform nursing duties including treatments, medication delivery, and resident emergency management.
• Coordinate prescription orders and oversee pharmacy delivery and MARs.
• Assist in coverage during absences and maintain compliance with policies.
