Job summary

MBK Senior Living is seeking a Health Services Coordinator (LVN/LPN) to support the health services team. You will assist with assessments, develop care plans, manage medication delivery, and coordinate with doctors and families to ensure quality resident care.

This role requires active nursing licensure (RN/LVN/LPN), strong organizational skills, and at least 2 years of healthcare experience. A comprehensive benefits package is offered in a full-time role based in Huntington Beach, CA.

Qualifications

• Active RN/LVN/LPN license in good standing is essential.
• Minimum 2 years of related healthcare experience.
• Strong administrative and clerical skills with attention to detail.

Responsibilities

• Coordinate assessments and recommendations for admission.
• Draft and update Individualized Service Plans (ISP).
• Review service plans with DHS and responsible parties.
• Maintain continuity of the resident care regimen.
• Deliver medication and oversee safety protocols and documentation.
• Coordinate prescriptions and pharmacy delivery with doctors’ offices.
• Assist in training on essential topics including medication delivery and state requirements.
• Promote teamwork and MBK core values in daily operations.
• Perform other duties as assigned by Supervisor or Executive Director.
