Job summary

EMPACT- Suicide Prevention Center is looking for a full-time Case Manager in Apache Junction, AZ, to work with adults suffering from severe mental illness. You will provide crisis intervention services and support to families while ensuring continuity of care. Candidates must have a bachelor’s degree in behavioral health and experience with this population.

Qualifications

• Must have a bachelor's degree in behavioral health or meet BHT requirements.
• Experience working with Severely Mentally Ill Adults is required.
• Must have a valid AZ driver's license with no major infractions.

Responsibilities

• Provide direct service crisis intervention and support to families.
• Complete family needs assessments and make appropriate referrals.
• Ensure continuity of care in the provision of comprehensive services.
