Job summary

Attain Behavioral Health seeks a Group Facilitator to lead Therapeutic Behavioral Services groups in partner nursing homes. LSW or LPC or equivalent Ohio credential required; experience with older adults and behavioral health groups preferred.

Daytime schedule with collaboration with facility staff and our clinical team. You will complete group notes, Medicaid paperwork, and support resident engagement in a compassionate, person-centered approach.

Qualifications

• Must hold LSW, LPC, or equivalent Ohio credential.
• Experience facilitating behavioral health or substance-use groups.
• Experience with older adults, long-term care, or medically complex populations preferred.
• Strong communication, empathy, and ability to connect with residents patiently.
• Able to work independently in off-site facilities.
• Reliable transportation.
• Must meet Ohio Medicaid and background check requirements.

Responsibilities

• Facilitate TBS groups to support emotional health and quality of life for nursing home residents.
• Adapt group content for older adults and cognitive/mobility needs.
• Collaborate with facility staff and on-site clinical team to ensure consistent care.
• Complete group notes, progress updates, and Medicaid paperwork on time.
• Encourage resident participation and maintain a safe, respectful group environment.
• Participate in supervision and professional development as part of the team.
