Job summary

Attain Behavioral Health is seeking a Group Facilitator to lead Therapeutic Behavioral Services groups in partner nursing homes, focusing on emotional wellness and meaningful engagement for older adults.

This full-time role requires QMHS III, LSW, or LPC credentials, strong communication and empathy, and the ability to work independently in off-site facilities with on-site supervision and growth opportunities.

Qualifications

• Must hold QMHS III, LSW, LPC, or equivalent Ohio credential.
• Experience facilitating behavioral health or substance-use groups.
• Experience with older adults, long-term care, or medically complex populations preferred.
• Strong communication, empathy, and patient connection.
• Able to work independently in off-site facilities.
• Reliable transportation.
• Must meet Ohio Medicaid and background check requirements.

Responsibilities

• Facilitate TBS groups to support emotional health, coping, motivation, and quality of life for nursing home residents.
• Adapt group content for cognitive and emotional needs of older adults.
• Collaborate with facility staff and on-site clinical team to ensure consistent care.
• Complete documentation (group notes, progress updates, Medicaid paperwork) accurately and on time.
• Encourage participation, foster connection, and create a safe, respectful group environment.
• Participate in ongoing supervision and professional development.
