Job summary

• Able to work independently in off-site facilities.
• Reliable transportation required; must meet Ohio Medicaid and background check requirements.

Responsibilities

• Facilitate meaningful TBS groups that support emotional health, coping skills, motivation, and quality of life for nursing home residents.
• Adapt group content to meet cognitive and emotional needs of older adults, including those with mild cognitive impairment.
• Collaborate closely with facility staff and on-site clinical team to ensure residents receive consistent, compassionate care.
• Complete documentation (group notes, progress updates, Medicaid paperwork) accurately and on time.
• Support resident engagement by encouraging participation, fostering connection, and creating a safe, respectful group environment.
• Participate in ongoing supervision and professional development as part of a supportive behavioral health team.

What You'll Do

• Must meet Ohio Medicaid and background check requirements

Preferred Skills & Attributes

• Warm, compassionate approach with a genuine interest in the wellbeing of older adults

• Experience with EHR systems (training available)

• Trauma‑Informed Care training or willingness to obtain within 90 days

• Comfortable in nursing home environments and able to walk/stand as needed
