Attain Behavioral Health is seeking a Group Facilitator to lead Therapeutic Behavioral Services groups in nursing home settings near Newark, OH. You will facilitate engaging, person-centered sessions for residents, collaborate with facility staff, and handle Medicaid paperwork to support emotional well-being.

Must hold QMHS III, LSW, or LPC credentials; experience with older adults and long-term care preferred. Full-time role with independent off-site work and mileage reimbursement.

Qualificações

• Must hold one of: QMHS III, LSW, LPC, or equivalent Ohio-recognized credential.
• Experience facilitating behavioral health or substance-use groups.
• Experience with older adults, long-term care, or medically complex populations preferred.
• Strong communication, empathy, and ability to connect with residents patiently.
• Able to work independently in off-site facilities.
• Reliable transportation and ability to meet Ohio Medicaid and background check requirements.

Responsabilidades

• Facilitate TBS groups that support emotional health, coping skills, and quality of life for nursing home residents.
• Adapt content for cognitive and emotional needs of older adults with medical complexities.
• Collaborate with facility staff and on-site clinical team for consistent care.
• Complete documentation (group notes, progress updates, Medicaid paperwork) accurately and on time.
• Encourage participation, foster connection, and maintain a safe, respectful group environment.
• Participate in ongoing supervision and professional development with the team.
