Attain Behavioral Health seeks a Group Facilitator to lead Therapeutic Behavioral Services groups in partner nursing homes, using a person-centered approach to support emotional wellness for older adults.

The role requires Ohio-credentialed clinician (LSW/LPC or QMHS III or equivalent), strong communication, and ability to work independently in off-site facilities; transportation required.

Formación

• Must hold QMHS III, LSW, LPC or equivalent Ohio credential.
• Experience facilitating behavioral health or substance-use groups.
• Experience with older adults, long-term care, or medically complex populations preferred.
• Strong communication, empathy, and ability to connect with residents patiently and respectfully.
• Able to work independently in off-site facilities.
• Reliable transportation required; must meet Ohio Medicaid and background check requirements.

Responsabilidades

• Facilitate meaningful TBS groups that support emotional health, coping skills, motivation, and quality of life for nursing home residents.
• Adapt group content to meet cognitive and emotional needs of older adults, including those with mild cognitive impairment.
• Collaborate closely with facility staff and on-site clinical team to ensure residents receive consistent, compassionate care.
• Complete documentation (group notes, progress updates, Medicaid paperwork) accurately and on time.
• Support resident engagement by encouraging participation, fostering connection, and creating a safe, respectful group environment.
• Participate in ongoing supervision and professional development as part of a supportive behavioral health team.
