Attain Behavioral Health in Sandusky, OH is seeking a Group Facilitator to lead Therapeutic Behavioral Services (TBS) groups in partner nursing homes. The role requires LSW, LPC, or equivalent, and focuses on emotional wellness, coping, and relationship-building with older adults.

You will adapt content for cognitive needs, document progress and Medicaid paperwork, collaborate with staff, and participate in supervision within a supportive team.

Qualifications

• Credential required: LSW, LPC, or equivalent per Ohio regulations.
• Experience facilitating behavioral health or substance-use groups.
• Experience working with older adults, long-term care, or medically complex populations preferred.
• Strong communication, empathy, and ability to connect with residents patiently and respectfully.
• Ability to work independently in off-site facilities.
• Reliable transportation.
• Must meet Ohio Medicaid and background check requirements.

Responsibilities

• Facilitate TBS groups to support emotional health, coping, motivation, and quality of life for nursing home residents.
• Adapt group content to cognitive and emotional needs of older adults, including those with mild cognitive impairment.
• Collaborate with facility staff and on-site clinical team to ensure consistent, compassionate care.
• Complete documentation (group notes, progress updates, Medicaid paperwork) accurately and on time.
• Support resident engagement by encouraging participation and creating a safe, respectful group environment.
• Participate in supervision and professional development within a supportive behavioral health team.
