Job summary

Attain Behavioral Health seeks a Group Facilitator to lead therapeutic Behavioral Services groups in partner nursing homes. You will guide emotionally focused activities for older adults, collaborating with on-site teams to ensure high-quality care.

The role emphasizes compassionate engagement, documentation, and growth within a supportive behavioral health team, with daytime hours typical for group sessions.

Qualifications

• Must hold LSW, LPC, or Ohio credential.
• Experience facilitating behavioral health or substance-use groups.
• Experience with older adults, long-term care, or medically complex populations preferred.
• Reliable transportation and ability to work independently.
• Willingness to meet Medicaid and background check requirements.

Responsibilities

• Facilitate TBS groups supporting emotional health, coping skills, motivation, and residents' quality of life.
• Adapt content to cognitive/emotional needs of older adults, including mild cognitive impairment.
• Collaborate with facility staff and on-site clinicians to ensure consistent, compassionate care.
• Complete group notes, progress updates, and Medicaid paperwork accurately and on time.
• Encourage participation, foster connection, and maintain a safe, respectful group environment.
• Participate in supervision and professional development with the behavioral health team.
