Job summary

The candidate must hold QMHS III, LSW, or LPC or an equivalent Ohio-recognized credential and have experience with behavioral health groups. Travel to off-site facilities and reliable transportation are required.
Qualifications
- Experience facilitating behavioral health or substance-use groups.
- Experience working with older adults in long-term care or medically complex populations.
- Ability to work independently in off-site facilities.
- Reliable transportation and ability to meet background check requirements.
Responsibilities
- Facilitate TBS groups to support residents' emotional health and quality of life.
- Adapt content for cognitive and medical needs of older adults.
- Collaborate with facility staff and on-site clinical team for consistent care.
- Complete group notes, progress updates, and Medicaid paperwork accurately and on time.
- Encourage participation and maintain a safe, respectful group environment.
- Participate in supervision and professional development.
