Job summary

Amity Foundation in Los Angeles seeks a Case Manager to join the Enhanced Care Management (ECM) program. You will coordinate medical, behavioral, and social services for students with complex needs, aiming to reduce hospital visits and support housing goals.

The role requires field movement, collaboration with the ECM care team, and strong advocacy for students’ access to resources within the community. Local travel is expected; on-site work is required.

Qualifications

• Bachelor’s degree in social work, psychology, public health, or related field or associate’s degree.
• And minimum of two years’ experience in client-related field, providing case management and community outreach.
• The equivalent of a high school diploma and three cumulative years of outreach, reentry services, and case management.
• Strong communication and interpersonal skills, capable of building relationships with students, families, team members, and partners.
• Local travel and ability to navigate public transportation with students.

Responsibilities

• Field-based ECM Care Team member bridging between students and healthcare system.
• Engage ECM students who are hard to reach, addressing barriers to care.
• Assist RN Care Manager, BH Care Manager, and Care Coordinator in panel management.
• Support enrollment, appointments, and engagement in clinic and community settings.
• Help students navigate healthcare and social services.
• Follow up by phone and in person with ECM students.
